how is the background around the head made bright. Is it painted background, lighting, or digitally manipulated.

It is in the backdrop. It is called a hot spot. You can also get that effect by placing a light in front of the BD and behind your client or behind the bdrop for a filtered light effect. If you don't have one you can add a vignette around your model and it will give that effect. Put the vignette on a separate layer and use the soft light overlay mode in photoshop.
